Hendrix This document guides plant personnel in gathering data and samples in support of analyzing the cause for an equipment or piping system component failure when the failed sample cannot be promptly submitted to a qualified laboratory for a proper failure analysis. It is also includes useful information for a root cause failure analysis. The procedures and guidelines presented herein are applicable to all types of failures; however, they are primarily meant to address failures of fixed equipment, piping and piping components, and rotating equipment, and include information applicable to metals and non-metals. The "how-to" section includes procedures and guidelines applicable to most damage mechanisms.
